Step 1 - Payroll Information Request
In order to complete your 360 Preventative Health Plan RFP (Request For Proposal) we will need your client's Payroll Census Spreadsheet completed. Access the link below to download the Payroll Census Spreadsheet, which will need to be completed for all active full-time employees. Obtaining complete and accurate data is important to help us determine an accurate proposal.
✅ Forward the "Payroll Census Spreadsheet" to your client's payroll provider so they can quickly generate the report we need.
There is a tab labeled "Census Key" to help clarify required information.
✅ Upon receiving the completed census and/or any payroll reports, use the form and SECURE UPLOAD link below to complete your RFP.
The use of the secure upload link will keep all information secure and within HIPAA guidelines.

REQUIRED INFORMATION:
Depending on the accounting software and/or payroll software used, the title of the type of the report(s) may differ. The report needed should have the following information: all W-4 information including Name, SSN, DOB, Address, Phone Number, E-Mail Address, Marital Status, and Dependents. In addition, reports needed will include Wage Information (Salary or Hourly Earnings), date of hire, including any pretax deduction types and/or wage garnishments.
The OACEUS census is a template form it is optional to fill out, this census is to be used as a guide for ALL REQUIRED INFIORMATION.(Information may be found on multiple reports)
Step 2 - Complete Your Client Information Below
If you're a producer that is W2 under an Agency, you must enter your "Agency ID #" in the form below.
If you're a producer that's 1099 under an Agency, you must enter your Agency ID #, and Producer ID #.
